Position Summary:
• The CNA Program Assistant will provide personal care, medication administration, and monthly wellness vitals/weights for participants with Registered Nurse oversight, assist the Adult Day Service Program Manager and Activity Coordinator with projects.
Duties And Responsibilities:
• Assist clients in arriving and leaving program site. Including removal of coats, wellness screening, assisting with walkers and wheelchairs.
• Assists Activity Coordinator with activities, special events and parties.
• Assist with taking clients for walks and outings.
• Socializes with clients and participates in 1 on 1 activities with clients.
• Reports unusual behavior or changes in behavior/health of clients (disposition, wandering, illness) to Manager or Nurse
• Assists with toileting/bathroom hygiene of clients per client’s individual care plan.
• Informs Manager of all schedule changes within an appropriate amount of time, i.e., calls in sick as soon as possible.
• Communicates in an appropriate manner with clients, caregivers, case managers, staff and volunteers.
• Daily documentation of activities, behaviors, wellness, and toileting
• Participates/leads Health and Wellness Activities
• Administers medication per the Medication Administration Record for each participant
• Conducts monthly vitals and weight for each participant, and as needed
• Maintains communication with the Nurse regarding client’s well-being and safety concerns
• Provides input into Initial, Annual, and Quarterly Care Plan reviews
• Maintains the Wellness Room, and Medication Administration Area
• Manage/initiate the daily functions/activities of Adult Day Services as needed
• Other duties as assigned.
Qualifications:
• Have a high school diploma or GED
• Certified Nursing Assistant or Medical Assistant certification preferred
• Demonstrate an aptitude in working with individuals affected by Alzheimer’s Disease, or other related dementias, and cognitive impairments.
• Must be responsible and work well with others.
• Experienced in working with the elderly, handicapped or dementia patients is preferred.
• Ability to complete multiple duties and tasks correctly and on time without being prompted.
• CPR and First Aid certified
